Caution: Some web browsers may open the Excel or PDF file within the browser window; we highly recommend that, before entering any data, you save the file to your computer and then open it using Excel or Adobe Acrobat, as you may find it difficult to save the file with your data from within your web browser.

Download the A.R.E. Standard Vendor Qualification Form.

Note: Adobe Acrobat (or a similar PDF forms program) is required to save, edit, and reuse the form.

Be aware that Adobe Reader will allow you to fill out and print the form, but will not allow you to save the form with your additions.

To download files to your computer's hard drive:

Mac: From your browser, click on the link to the forms and hold down the mouse button. A window will pop up, asking you to "Save link as..." (Netscape) or "Download Link to Disk" (Internet Explorer). If you are using Safari, control-click on the link and choose "Download Linked File." If the file opens in a window in Safari, you can type into the form and choose "File: Save As."

PC: From your browser, right-click on the link to the forms. A window will pop up, asking you to "Save link as..." (Netscape), "Save Target As" or "Download Link to Disk" (Internet Explorer).
